If you ship to local and long-haul zones at the same flat rate, near customers overpay and far customers underpay. Either (a) charge actual calculated shipping at checkout, (b) use flat rate equal to weighted average, or (c) build shipping into product price and advertise "free shipping."
Count packaging separately — mailer, tape, void fill, printed label, thank-you note, tissue. It's usually $0.75-$1.50/order that nobody remembers to track.
